Please submit a completed form for any truant students. Your may return the form via email to truancy@s-cook.org or you may fax the form to us at the fax below. Upon initial receipt, a first letter will be mailed to the home of the student. If the truancy continues, a second and third notice will be sent to the student's home.

A separate TRUANCY REFERRAL FORM is required each time, indicating the additional days truant and all additional corrective actions the district has initiated to remediate the truancy.
